GRAND RAPIDS, Mich. (WOTV)- Memory care facilities, like Samaritas, recognize that no two dementia patients are alike, and will offer personalized care for each individual.

At the Woods at Samaritas, we put together an Individualized Care Plans for each resident. These care plans portray the likes and dislikes, social background and any special needs of the individual. This care plan also includes the patient’s level of care and displays where the individual may need more assistance in, such as help during meals, using the facilities, bathing, medication delivery preference, and much more.

We encourage family members to be open about providing information about your loved one ahead of time to enhance our care plan. If we are aware of your loved ones hobbies and interests, it will help us build a relationship, which will put your family member at ease with their new situation. This also aids our staff in helping your loved one make new friends with similar interests. It is beneficial to have someone to relate and talk to, to ease the transition to the memory facility.

Samaritas begins this care plan prior to your loved one moving in. Prior to admission, we like to meet with the resident as well as their family outside of Samaritas. This gives us a great opportunity to see the individual in their current living space. We can see their personality portrayed through the paint colors, decorations and family photos in their home. As we get to know the resident within their new living arrangement, we build on their care plans as well to ensure they are always updated.
